If you are preparing materials to hand out to each person you will need to prepare 55 pieces to cover Mohawk inside & outside sales representatives.

Vendor Payment Information 

Payment by check is preferred. Invoices upon request to help collect payment by check, credit card or credit memo (only Mohawk payment eligible for credit memo).

If you elect to make a sponsorship please add on that total sponsorship amount to the total payment you make to Worthwhile Life Foundation
example: John Smith Company is sponsoring $1,500 in addition to his mandatory contributiuon of $1200, John Smith Company owes $3,700 to Worthwhile Life Foundation and $2,000 to Mohawk Rubber Sales.
